FEES AND BILLING PRACTICES
MWPR has a set fee schedule that is consistent with governmental guidelines suggested by the Health Care Financing Administration (HCFA), reimbursement schedules of most insurance companies, and with those charged by other local psychologists, mental health counselors, and psychiatric nurse practitioners. Our relationship with specific insurance companies require certain adjustments to be made. Please contact our office to discuss fees and billing issues further.
Our fees may change depending on reimbursement schedules and local adjustments, all changes will be updated here at the time of change.

AS SUCH, WE PROMISE TO SERVE ALL PATIENTS, OFFER DISCOUNTED FEES FOR PATIENTS WHO QUALIFY, AND DO NOT DENY SERVICES BASED ON A PERSON'S RACE, COLOR, AGE, SEX, NATIONAL ORIGIN, DISABILITY, RELIGION, GENDER IDENTITY, SEXUAL ORIENTATION, INABILITY TO PAY, ETC.
SLIDING FEE DISCOUNT PROGRAM
Mountain West Psychological Resources (MWPR) is committed to providing affordable and quality services to our community. Our Sliding Fee Discount Program (SFDP) is designed to provide no cost or discounted cost to those who have fewer means to pay for their psychiatric and/or behavioral health services. The SFDP is available to all individuals and families with annual income at or below 200 percent of the most current Federal Poverty Guidelines. Participation in this program is solely determined by a patient’s family size and income.
An application may be requested and submitted by the patient, a family member, a social-services staff, a person on their medical/health care team, or others that are aware of the patient’s existing financial hardship. Patient will need to provide a signed consent for your provider or any designated person to discuss any and all matters with those requesting and submitting the application.
Information and forms can be obtained from the provider directly – as well as the Patient Account Representative or Receptionist at Mountain West Psychological Resources via in-person, by telephone, or mail when requested. For convenience, the application (which includes the fee schedule) is posted below as well.
