No-Show and Late Cancellation Policy

Mountain West Psychological Resources is committed to providing exceptional care to all
clients. To do so, we need to ensure that scheduled appointment times are available to those who need them. The following policy outlines our guidelines for missed appointments and late cancellations.

Policy Details

1. No-Show Appointments
A "no-show" occurs when a client misses a scheduled appointment without providing notice. Clients who do not attend their appointment without prior communication will be charged a $100 no-show fee.

2. Late Cancellations
Appointments canceled with less than 24 hours' notice are considered late cancellations and will also incur a $100 fee.

3. Repeated No-Shows or Late Cancellations
Consistency in attending appointments is crucial for effective care. Clients with three (3) no-shows or late cancellations may have their care terminated with our clinic. This step ensures we can accommodate clients who consistently attend their scheduled sessions.

4. Exemptions
We understand that emergencies and unforeseen circumstances can occur. Exceptions to
this policy may be made at the discretion of your provider or clinic management. Please notify us as soon as possible if you cannot attend your appointment due to extraordinary circumstances.

5. Billing and Payment

  • The no-show or late cancellation fee is billed directly to the client.

  • Insurance companies do not cover fees for missed appointments or late cancellations; the client is responsible for the full amount.

  • Payment of outstanding fees is required before scheduling future appointments.


6. Appointment Reminders
As a courtesy, we provide appointment reminders, which can be customized via the patient portal. However, it is ultimately the client’s responsibility to remember their appointment date and time.

7. Rescheduling
If you need to reschedule, please contact us at least 24 hours in advance to avoid the late cancellation fee. Early communication allows us to offer the time slot to another client.

Sliding Fee Discount Program Application

SLIDING SCALE FEE PROGRAM

Mountain West Psychological Resources (MWPR) is committed to providing affordable and quality services to our community. Our Sliding Fee Discount Program (SFDP) is designed to provide no cost or discounted cost to those who have fewer means to pay for their psychiatric and/or behavioral health services. The SFDP is available to all individuals and families with annual income at or below 200 percent of the most current Federal Poverty Guidelines. Participation in this program is solely determined by a patient’s family size and income.

An application may be requested and submitted by the patient, a family member, a social-services staff, a person on their medical/health care team, or others that are aware of the patient’s existing financial hardship. Patient will need to provide a signed consent for your provider or any designated person to discuss any and all matters with those requesting and submitting the application.

Information and forms can be obtained from the provider directly – as well as the Patient Account Representative or Receptionist at Mountain West Psychological Resources via in-person, by telephone, or mail when requested. For convenience, the application (which includes the fee schedule) is posted below as well.
